Credit card payment by fax
To make your credit card payment by fax, follow these instructions

1. After adding all desired products to your basket, proceed to checkout.
2. Fill out all customer information as directed and proceed to payment screen.
3. Enter your credit card payment information and print out this screen. (Do not click the continue button.) Include information about any items showing "details" or requiring personalization.
4. Fax your completed form to: 866-592-8642 (Please include phone number and email for any questions regarding your order.)

To pay for your order by check, please follow these instructions
